Going through StumbleUpon and found this handy link. I'm tempted to print it out and hang it up at work.

That is true! They also forgot to add in the managers that play favorites, harass and/or bully employees and the lazy managers too!

While it didn't have a specific category, most of the result form communication problems. Managers expect the subordinates to know certain things but don't communicate that clearly and efficiency.
