Brain burp of a co-worker. She called an agency office to see about getting registered with their state for a product (report) we would like to be able to use. She's told she'll have to complete a form and return it. So, she asks them to fax her 2 forms since we have 2 doctors. Fast forward 10 minutes.......................................she got the form, once. She states, "Michelle, they only sent me one copy of the form!" 
I could only snicker since she just pulled the form off of the copy/fax machine. Duh, make a copy. She was so embarrassed!
