I remember back when I was a teenager, OSHA actually came out with a 17 page booklet (or something like that) on the proper use of a ladder!
At one of the last Penn Central RR Historical Society conventions, we all got to see one of the original training films. IIRC it ran for about 10 minutes or so, and covered the basics of not only crossing tracks, but how to look for moving equipment, and how to climb freight car ladders properly.
