Note: in Australia we drive on the left hand side of the road.
Fail #1:
In order to get to a shopping centre near my house, there's a back access road that stops at a roundabout between my street and the street nearby. If you were to look at it on Google maps, my street would be facing West, the other street would be facing north and south, while the back access road is facing East.
Guy decides to come tearing down the back access road, thankfully not hitting me. However, when he gets to the roundabout, instead of doing a 270 degree turn to go right, he decides to simply cut across the lane and do a 90 degree turn, ending up on the WRONG side of the road and luckily not causing an accident.
(for you US folk, I believe you'd do the 270 degree turn to go left and the 90 to go right, in Australia it's flipped)

Not much to add to this. Guy decides that he needs to do a U-Turn on a freeway.
