One McDonald's refused to make a McDLT with the hot side and cold side switched in the box.

McDLT? McDonut, Lettuce, and Tomato? That's much better than the only other thing I could come up with... (McRichard, etc.)

On topic, I don't remember ever seeing a knife at McD's when I worked there, everything was pre-sliced, there was a bun toaster... we had those worthless plastic knives in a wrapped package, but, still, they're worthless!
