      When I worked as the triage nurse, I would get Hispanic patients who didn't speak any English. Part of my assessment includes basic info: name, DOB. Unfortunately, I don't know how to say "what's your date of birth" in Spanish. So the patients just hand me their Medi-Cal cards.

      On more than one occasion I would note the name on the card didn't match what they had told me (I can say, como se llama). When I'd ask, they'd say, "Oh, that's my work name."

      Riiiggght.
      They say that God only gives us what we can handle. Apparently, God thinks I'm a bad ass.

        I have a photostat copy, black background with white type on shiny paper, of my birth certificate my parents received back in 1966 when my Mom moved from California to South Dakota. I also have the California long form birth certificate that I ordered since the PO said the Passport people might not accept the photostat copy and kick back my application (they accepted the photostat). <snip> My MIL sent us copies of my hubby's and daughter's certificates, she worked at a SD county courthouse so she could pull them up for free . They only had the basic info - baby's name, DOB, place of birth, mother's name and father's name on a 4x6 inch piece of green and white paper.
        I was born in New York City. There's two kinds of proof of birth you can get there. The first one is the standard birth certificate, which is the computer printout on the green paper with the raised seal that lists only the basic demographic facts, and the other is a "certified copy of the birth record", which is the kind you mention above. This is apparently enlarged from a microfilm of the actual document from the hospital where I was born. My mom's original copy was white print on greenish- black background, reduced in size from the original; when I ordered my own copy 18 years later, it came as black print on yellow background, and full-sized. I guess they improved the technology in the interim. Both had hand-applied raised impressions, but no gold foil.

        (I don't remember if it had my parents' occupations, but there was a lot more interesting info there than on the computer-printed one, including (e.g.) the signature of the MD who delivered me.)

        Right now we are having all sorts of hassles trying to get a birth certificate for our daughter, who was born in the house (she beat the ambulance by three minutes) and then taken to a hospital in the next county to the east. It seemed they aren't used to unplanned home births, because they sent a health inspector down to the house to interview us and gather all thedata about the birth. It finally dawned on me that they were trying to ensure that the baby was really ours, not one that we'd kidnapped, or worse, taken from a murder victim by impromptu Caesarean. (It's happened, and not all that long ago either.) Besides the question of which county issues the certificate. It looks at the moment that the county where the hospital was, will be the issuing agency; not sure what they'll put down for place of birth. Have to see it when it finally comes, after which I can finally get my daughter some health insurance, which you can't get without a birth certificate. Heck it's only been two months since she arrived...

          A couple of weeks before my brother's 19th birthday (19 being the drinking age in Canadia) a "friend" of his wanted to go out drinking. "But I'm not 19 yet," my brother protested. "No problem," his friend responded as he took my brothers license and began scratching at the birth year.
          So not only did they not get into the bar that day, when he finally did turn 19 two weeks later he STILL couldn't get in because his ID had been tampered with. :P
          It was months before he finally got a new ID. Oh how I teased him mercilessly.
