It's possible that he has an invisible disability, and is in denial about it.
Now, having the disability is one thing - and as almost everyone has said, a polite request to have the kitchen or the wait staff cut a steak is no problem. (Ideally, without having to admit to disability - just being polite.)
But some people who are in denial about their disability can be very angry, rude, even aggressive about it. And no, the general public shouldn't have to suffer that anger and rudeness. But it could well explain what happened.
If it is that, I hope the guy gets the help he needs - both with his disability and with his reaction to it. And that he damn well tips wait staff well until he gets over it.
