PS: Plain == regular, it means the way it normally is, plain, nothing extra, no changes. It doesn't mean modifications!
If I gave one of my customers a normal hamburger when they told me a plain hamburger, there would be hell to pay! Regular means regular, plain means a variety of things.

In the grand scheme of things, a worker at a Taco Bell may not be important but in that particular person's life at the moment, the fact that you are there meant that he gets his food.

When I was still working in the world of "fast service dining", I know I would always double-check with the customer when they asked for something as "plain". Of course, you will always get someone who can't conceive that some folks might use (or misuse) the term "plain" to mean something else...

I would never, EVER use the word "plain" to order food (unless that was a specifically-defined option), it's just too vague >_>

If I want a burger with naught but ketchup, I'll say 'ketchup only'

If I don't want cheese, I'll say 'no cheese'

If I want it cooked for precisely 3 minutes and 5 seconds, with ketchup, light mayo, 2 kinds of cheese (with something else in between them), no veggies, double bacon with half on top and half underneath the patty, extra gravy, and a toasted bun, I will, of course, say "Oh, ya know. The usual" and drive up to pay

Ah yes. The miracle of writing down what you intend to order, precisely as you intend to order it, and giving that to the cashier. It cuts out 85 % of this bullcrap.
Customers should always be served . . . to the nearest great white.
