I never even thought about how some people care about this until after the '92 riots when LA talkradio mentioned that it was a cultural practice of--I think--Korean shopkeepers to put change on the counter (I guess they were scraping for explanations as to why some inner city folk found them alienating in the wake of their stores being targets
).
And as I was reading this thread, I couldn't particularly recall how I generally hand over money--it varies with the clutter and the person and my preparedness, but I prefer to have it on the counter so they can take it when they're ready. I guess not everyone puts stock in anticipation because my office had software which was set to move the cursor back to the upper left corner when the screen changes instead of leaving it at the spot I move it to for my next action
--anyway, I just know to be careful that no one else can grab the presidents resting unencumbered). 
I do remember finding it awkward when cashiers made a point of handing me my receipt, bills and coins all together in one palm, instead of placing them on the counter so I could more easily separate and file them properly (especially when my other hand was holding something else). And I'd assume cashiers would prefer the counter placement for the same reason--guess not.
