Back when my parents were in school there was a place here that charged you extra (on the order of 5 cents) if you got your coffee with milk and/or sugar.
We have (had?) a restaurant locally that did (does?) that. Not to mention $0.25 per pat of butter. I'm not sure if this is still the policy because I walked out when they started doing it and never went back. They are still in business, so maybe they wised up. I can't see their ususal clientele (blue-collar workers and retirees) putting up with it for long when there's alternatives within walking distance....

There are a few diners around here, mostly of the greek food variety, that seems to charge for every little thing. Sodas are 1 free refill that are packed with crushed ice, and woe to you that orders it without ice. If you order a sandwhich, there are no chips that come with it. It is your sandwhich and just your sandwhich with maybe a dinky pickle as well. Your sides are obviously extra, running about 3-4 dollars for a tiny plate of fries, or in my case mashed potatoes.

I don't personally like going to those places because of that, and it's not all that great food. But 24 hour diners, you get what you can.
